To request a new account:

1. Go to https://www.docusign.com.
2. In the upper right-hand corner select Log In [Note: do not select FREE TRIAL].

4. Then you will be taken to the Rutgers CAS login page to complete.
5. Log in to your new DocuSign account at https://account.docusign.com/.
6. Visit (https://it.rutgers.edu/docusign/) for training and support resources in the Resources section.

REQUIRED FORMS FOR PHD STUDENTS

Lab Rotations

• Lab Rotation Form (.pdf)
• Students will find documents and links to useful information that will help navigate the rotation process in the assigned Lab Rotation Canvas course sites. 
• All first-year Molecular Biosciences students must rotate in three laboratories of faculty members affiliated with Molecular Biosciences programs, and therefore will need to complete three separate Lab Rotation forms.
• Form submission instructions will be provided in the assigned Canvas course site.

Thesis Advisor Selection

• Thesis Advisor Selection Form
• You and your thesis advisor are asked to read, and acknowledge that you have read, the AAMC Compact for Biomedical Graduate Students and Their Research Advisor and the AAMC Appropriate Treatment of Research Trainees
• Form submission instructions will be provided in the assigned Lab Rotation Canvas course site.

Qualifying Exam A

• There are no forms required. All information will be provided in the assigned Canvas course site.

Qualifying Exam B

• Qualifying Exam B form (Proposition)
• Research Advisory Committee Meeting Form

Annual Research Advisory Committee Meeting

Individual Development Plan (IDP)

• All forms and information will be provided in the assigned Canvas course site.

Final Dissertation and Graduation

• Please follow the checklist and complete all forms by the designated deadlines in accordance with the School of Graduate Studies PhD Graduation and Forms website.
